EMPLOYMENT VERIFICATION
Employees may request VOE directly or vendors may request on employees’ behalf. Verifications will be mailed, faxed or e-mailed, within 48 business hours, to the business/organization requesting verification. Employees may also obtain a verification by visiting the Human Resources Benefits & Records Office, and completing the Request for Verification of Employment form. Verifications include employee name, job title, dates of employment and salary. Requests may be faxed to 318-675-7990 or emailed to shvbenefits@lsuhs.edu.
Form 1095-C Availability
LSU Health Sciences Center-Shreveport provides Form 1095-C in accordance with IRS requirements.
Active employees may access their Form 1095-C electronically through Employee Self-Service.
Paper copies of Form 1095-C are not automatically mailed. However, any employee or retiree may request a paper copy of their Form 1095-C at no cost.

Requested forms will be furnished no later than January 31, 2026, or within 30 days of the request, whichever is later.
